My nomination for gayest album cover is Extra Fancy, the LA punk band with a gay lead singer. The cover of their single "You Look Like a Movie Star, Honey", sports an illustration of John Travolta in his traditional "Saturday Night Fever" disco-king pose, but naked and with a huge penis. I have it in my collection. It's VERY gay. And, yes, they got in a lot of trouble over it.
